Following are the fields that you need to fill for the I-94 number retrieval

Family Name- Enter your Last name on family name as per passport.
First (Given) Name- This is your name. Don't include middle name.
Birth Date- DOB should be in the format of MM/DD/YYYY. You can pick it from the pop up from 'Select' option beside the field.
Passport Number- Enter your passport number.
Country of Issuance- Place where your passport is issues. This can be found on passport information page. For Example: Hyderabad is the usual place for people from Andha Pradesh.
Date of Entry- The date you recently entered into USA. This can be found from stamp on passport given by CBP.
Class of Admission- This is the visa status. For students it is F1 and for workers it is H1B. There are many other categories. Don't confuse between F1 and F11. Students from India come to US on F1.

Once you fill the from and submit it you can see
1) Admission Record (I-94) number
2) Admit until date: Data you need to leave USA.
3) Details provided on I-94 Admission Form.
